α-Pirrolidinopentiofenona (α-PVP) en el Contexto del Chemsex

Introducción. La α-pirrolidinopentiofenona (α-PVP), conocida en el ámbito callejero como «Flakka» o «Alpha», es una catinona sintética de segunda generación. En Europa, su consumo se ha vinculado estrechamente al fenómeno del Chemsex, sobre todo entre hombres que tienen sexo con hombres (HSH). Su potente efecto dopaminérgico, una farmacocinética de acción rápida y su capacidad para desencadenar cuadros de delirio agitado la convierten en una sustancia de alta criticidad para la medicina legal y la criminología actuales.
Objetivos. Este trabajo busca describir las propiedades farmacológicas y toxicológicas de la α-PVP y analizar su implicación en muertes de varones de mediana edad en entornos de Chemsex. Asimismo, se proponen pautas de actuación médico-legal ante intoxicaciones y agresiones sexuales facilitadas por drogas (DFSA) y se revisan las metodologías analíticas actuales para su detección.
Métodos. Se realizó una revisión sistemática narrativa de la literatura científica publicada entre 2018 y 2025 en las bases de datos PubMed, Scopus y Web of Science. Esta búsqueda se complementó con informes institucionales de la Agencia de la Unión Europea para las Drogas (EUDA, antigua EMCDDA) y de organismos forenses europeos de referencia.
Resultados. La α-PVP opera como un inhibidor potente y selectivo de los transportadores de dopamina (DAT) y norepinefrina (NET). En estos transportadores, su potencia llega a superar hasta 100 veces a la de la cocaína, mostrando una actividad serotoninérgica casi nula. Las principales causas de muerte directa documentadas son el infarto agudo de miocardio y el edema pulmonar agudo, precedidos habitualmente por delirio agitado e hipertermia maligna. En las prácticas de Chemsex, su uso suele ligarse al slamming (inyección intravenosa) y a escenarios de sumisión química. Actualmente, la técnica LC-MS/MS es el método de referencia para confirmar su presencia en muestras biológicas post mortem.
Conclusiones. La α-PVP plantea un desafío creciente para los entornos sanitario, forense y judicial. Resulta prioritario capacitar específicamente a médicos forenses, toxicólogos y fuerzas de seguridad, además de estandarizar los protocolos de toma de muestras y la cadena de custodia en escenarios de Chemsex.
